AST Blood Test: What It Measures and When You Need One

What aspartate transaminase is

Aspartate transaminase is an enzyme involved in amino acid metabolism. It sits in high concentrations in the liver, but also in the heart, skeletal muscle, kidneys, and red blood cells. When cells in any of these tissues are damaged, AST leaks into the blood.

Because AST comes from several places, a raised result is less specific than a raised ALT, which is more liver focused. Giannini et al. (2005) noted that doctors usually read the two enzymes together to work out which organ is the likely source.

Why an AST test is done

AST is part of standard liver and metabolic panels. It's ordered to assess liver health, to look into symptoms of liver stress, or to monitor a known condition such as hepatitis.

It has a history in cardiac assessment too, though troponin has largely replaced it for heart muscle injury. Melanson et al. (2007) confirmed troponin as the preferred marker there. AST still appears when evaluating muscle disorders and as part of routine panels, pre medication monitoring, and follow up after an abnormal result.

Normal AST levels

A normal AST usually falls between 10 and 40 units per litre, with some variation by laboratory and sex. Men tend to run a little higher than women.

Levels up to three times the upper limit are mildly raised. Above ten times normal suggests significant tissue damage and needs prompt investigation.

What raises AST

Liver related causes include alcohol related and non alcoholic liver disease, viral hepatitis, and medication effects. Non liver causes include heart strain, muscle injury or inflammation, strenuous exercise, and breakdown of red blood cells.

Some medications, including statins and certain antibiotics, can raise AST. Russo et al. (2004) documented drug related liver enzyme elevation and noted that over the counter products and supplements are often overlooked. A hard workout can lift AST for a day or two through muscle involvement, so mention recent intense exercise when you review results.

The AST to ALT ratio

Reading AST and ALT together is more informative than either alone. Williams and Hoofnagle (1988) described how a ratio above 2 to 1 often points toward alcohol related liver stress, while a ratio below 1, with ALT above AST, is more typical of fatty liver or viral hepatitis. Your doctor uses this ratio as one clue among symptoms, other markers, imaging, and history.

Is AST or ALT the better liver test?

They measure different things and work best together. ALT is more specific to the liver. AST adds information, especially through the AST to ALT ratio.

Can exercise raise my AST?

Yes. AST is present in muscle, so intense exercise in the day or two before a test can lift it. Tell your doctor if you trained hard recently.

Do I need to fast before an AST test?

AST alone doesn't require fasting, but it's often drawn with a panel that does. Follow the instructions for the full set of tests.

What if only my AST is high and ALT is normal?

That pattern can point away from the liver, toward muscle or other sources. It's interpreted alongside the ratio, your symptoms, and history rather than on its own.
